I can only repeat what I have said before. You need to evidence that you have worked in accordance with the visa that was issued - performing the stated role with the salary required by the visa and the current SOC. The form gives some very strong guidance with regard to what is required and you should read it carefully and then ask specific questions rather than asking for full application guidance.
